GAO Member Clubs are invited to submit Hole-in-One Certificates requests online.
If a member or guest (individual must be a GAO member) achieves a Hole-In-One at your club please complete the certificate and pin request form below. (Note all submissions must be attested by a Club Official).

Once your request is received and verified, Hole-in-One certificates and pins will be sent to the Member Club for distribution at the beginning of September.
